PURPOSE OF THIS FORM
This form is to be used to add your guests to an Autism on the Seas Staff Assisted Cruise, while retaining the booking under your agency.
AotS Staff Assisted Service Fee
The Staff Assisted Cruise Service Fee (Staff Assisted Cruise List) will be Invoiced by AotS directly to your guests, and payment will be collected by AotS directly from your guests.
Immediate Family Members - REGULAR AotS Service Fee
APPLIES to Immediate Family Members: Parents, Caregivers, Guardians, Guests (of any age) with Special Needs, Siblings of Guests with Special Needs

Our Staff Assisted Cruises are open to all Extended Family and Friends. Extended family and friends can book with our Travel Partners and our Service Fee is REDUCED to just $30 per guest, and they also receive all of the same benefits and services, including expedited/priority boarding, private muster drill, dinner seating, private venue sessions, Staff coordinated Shore Excursions, reserved show seating, siblings and child friends can attend our respite sessions, etc, etc
NOTE: Extended Family & Friends will have their Service Fee Waived if they book direct with AotS.
